The Association between Vitamin D Receptor Expression and Prolonged Overall Survival in Breast Cancer.
Summary
In this study, we analyzed vitamin D receptor (VDR) expression and survival in a breast cancer patient cohort of 82 breast
cancer patients. Immunohistochemical analysis was possible in 91.5% of the patients (75/82). Staining was evaluated using the
semi-quantitative assay according to Remmele and Stegner (immunoreactivity score [IRS]). IRS 0–1 was negative/very low, IRS
2–4 was moderate to high, and IRS 6–12 was high. Statistical analysis was performed by Spearman’s correlation test (p<0.05
significant). Overall survival was analyzed using Kaplan-Meier estimations. Only 6 patients had a negative IRS. Moderate IRS
values were present in 20 patients. Most of the patients had a high IRS (49). For survival analysis, data were dichotomized
(IRS 0–4: negative to moderate and IRS 6–12: high VDR expression). In univariate analysis, VDR expression showed significant
differences in progression-free survival (PFS) and overall survival (OS). Patients with high IRS scores showed significantly better
PFS and OS than patients with moderate/negative IRS scores for VDR expression. Tumor size was significantly correlated to
PFS. When analyzed separately, the three different IRS groups showed significant differences in VDR expression. The present
data suggest that VDR expression in breast cancer tissue may be of clinical significance, and the results provide evidence that
VDR may be a factor with prognostic relevance. (J Histochem Cytochem 60:121–129, 2012).

Molecular diagnosis of multiple endocrine neoplasia type 2A
Objective. To identify by means of genetic analyses individuals who are at risk of developing medullary thyroid cancer that is a component of multiple endocrine neoplasia.Subjects. A three-generation kindred with clinically and biochemically diagnosed medUllary thyroid cancer.Method. Identification of a heterozygote mutation by nucleic acid sequencing and restriction analyses.Results. A heterozygote T → C (Cys → Arg) mutation at codon 618 in exon 10 of the RET proto-oncogene was identified in 4 family members who had previously been diagnosed with medullary thyroid cancer. The same mutation was also found in one of the proband's presymptomatic children who subsequently underwent a preemptive thyroidectomy. The genetic diagnosis was confirmed by histology. No mutations were detected in any other family members.Conclusion. Identification of heterozygote germline mutations in multiple endocrine neoplasia is direct, highly accurate and cost-effective. This study demonstrates that, appropriately used, molecular diagnosis can supersede conventional biochemical methods in the management of patients with inherited cancers
Understanding graft-versus-host disease. Preliminary findings regarding the effects of exercise in affected patients
Advances in this century regarding allogeneic hematopoietic stem cell transplantation (allo-HSCT) have led to an expanding population of long-term survivors, many of whom suffer severe side effects, particularly those related to graft-versushost disease (GVHD), a potentially multi-systemic disorder caused by immunoeffector donor lymphocytes that destroy host tissues. The GVHD, especially in its chronic form (cGVHD), generates considerable morbidity and compromises the physical capacity of patients. We have reviewed the main pathophysiological aspects of the disease as well as the data available on the effects of exercise in GVHD, based on animal and human patient research. Although exercise training as an adjunct therapy to improve health outcomes after allo-HSCT shows promise (particularly, this lifestyle intervention can improve physical fitness and possibly immune function while attenuating fatigue), there is a need for more randomized control trials that focus specifically on GVHD
Gravitational Collapse and Fragmentation in Molecular Clouds with Adaptive Mesh Refinement
We describe a powerful methodology for numerical solution of 3-D
self-gravitational hydrodynamics problems with extremely high resolution. Our
method utilizes the technique of local adaptive mesh refinement (AMR),
employing multiple grids at multiple levels of resolution. These grids are
automatically and dynamically added and removed as necessary to maintain
adequate resolution. This technology allows for the solution of problems in a
manner that is both more efficient and more versatile than other fixed and
variable resolution methods. The application of AMR to simulate the collapse
and fragmentation of a molecular cloud, a key step in star formation, is
discussed. Such simulations involve many orders of magnitude of variation in
length scale as fragments form. In this paper we briefly describe the
methodology and present an illustrative application for nonisothermal cloud
collapse. We describe the numerical Jeans condition, a criterion for stability
of self-gravitational hydrodynamics problems. We show the first well-resolved
nonisothermal evolutionary sequence beginning with a perturbed dense molecular
cloud core that leads to the formation of a binary system consisting of
protostellar cores surrounded by distinct protostellar disks. The scale of the
disks, of order 100 AU, is consistent with observations of gaseous disks
surrounding single T-Tauri stars and debris disks surrounding systems such as

Radio Emission from Ultra-Cool Dwarfs
The 2001 discovery of radio emission from ultra-cool dwarfs (UCDs), the very
low-mass stars and brown dwarfs with spectral types of ~M7 and later, revealed
that these objects can generate and dissipate powerful magnetic fields. Radio
observations provide unparalleled insight into UCD magnetism: detections extend
to brown dwarfs with temperatures <1000 K, where no other observational probes
are effective. The data reveal that UCDs can generate strong (kG) fields,
sometimes with a stable dipolar structure; that they can produce and retain
nonthermal plasmas with electron acceleration extending to MeV energies; and
that they can drive auroral current systems resulting in significant
atmospheric energy deposition and powerful, coherent radio bursts. Still to be
understood are the underlying dynamo processes, the precise means by which
particles are accelerated around these objects, the observed diversity of
magnetic phenomenologies, and how all of these factors change as the mass of
the central object approaches that of Jupiter. The answers to these questions
are doubly important because UCDs are both potential exoplanet hosts, as in the

Mood instability, mental illness and suicidal ideas : results from a household survey
Purpose:
There is weak and inconsistent evidence that mood instability (MI) is associated with depression, post traumatic stress disorder (PTSD) and suicidality although the basis of this is unclear. Our objectives were first to test whether there is an association between depression and PTSD, and MI and secondly whether MI exerts an independent effect on suicidal thinking over and above that explained by common mental disorders.
Methods:
We used data from the Adult Psychiatric Morbidity Survey 2007 (N = 7,131). Chi-square tests were used to examine associations between depression and PTSD, and MI, followed by regression modelling to examine associations between MI and depression, and with PTSD. Multiple logistic regression analyses were used to assess the independent effect of MI on suicidal thinking, after adjustment for demographic factors and the effects of common mental disorder diagnoses.
Results:
There are high rates of MI in depression and PTSD and the presence of MI increases the odds of depression by 10.66 [95 % confidence interval (CI) 7.51–15.13] and PTSD by 8.69 (95 % CI 5.90–12.79), respectively, after adjusting for other factors. Mood instability independently explained suicidal thinking, multiplying the odds by nearly five (odds ratio 4.82; 95 % CI 3.39–6.85), and was individually by some way the most important single factor in explaining suicidal thoughts.
Conclusions:
MI is strongly associated with depression and PTSD. In people with common mental disorders MI is clinically significant as it acts as an additional factor exacerbating the risk of suicidal thinking. It is important to enquire about MI as part of clinical assessment and treatment studies are required
Nanomechanical Detection of Itinerant Electron Spin Flip
Spin is an intrinsically quantum property, characterized by angular momentum.
A change in the spin state is equivalent to a change in the angular momentum or
mechanical torque. This spin-induced torque has been invoked as the intrinsic
mechanism in experiments ranging from the measurements of angular momentum of
photons g-factor of metals and magnetic resonance to the magnetization reversal
in magnetic multi-layers A spin-polarized current introduced into a nonmagnetic
nanowire produces a torque associated with the itinerant electron spin flip.
Here, we report direct measurement of this mechanical torque and itinerant
electron spin polarization in an integrated nanoscale torsion oscillator, which
could yield new information on the itinerancy of the d-band electrons. The
unprecedented torque sensitivity of 10^{-22} N m/ \sqrt{Hz} may enable
applications for spintronics, precision measurements of CP-violating forces,
